Patient satisfaction represents a key indicator for assessing the quality of hemodialysis services and is shaped by multiple factors, particularly service quality, psychosocial support, and patients’ trust in healthcare providers. This study was conducted to examine the influence of service quality and psychosocial support on the satisfaction of hemodialysis patients, while also exploring the mediating role of patient trust. The research took place at Karawang Islamic Hospital, where a decline in the patient satisfaction index has been observed in recent years. A quantitative research design was employed to investigate the relationships among service quality, psychosocial support, trust, and patient satisfaction. The study was carried out in July 2025 in the Hemodialysis Unit of RSI Karawang and involved 41 respondents selected through a total sampling technique. The findings demonstrated that service quality had a positive and statistically significant effect on both patient trust and patient satisfaction. Furthermore, trust was found to significantly mediate the relationship between service quality and patient satisfaction. Conversely, psychosocial support showed a significant direct effect on patient trust, but its effect on patient satisfaction was negative and not statistically significant. These findings highlight the importance of improving service quality and strengthening patient trust as strategic approaches to enhance satisfaction among hemodialysis patients
